Below you will find the Oklahoma State Department of Health Guide to Immunizations.   This guide details the immunizations required, by age,  for attendance in an Oklahoma public school.  To be eligible for an exemption from one or more of these immunizations, an official request must be approved by the Oklahoma State Department of Health. The process for getting an exemption from immunizations for the purposes of attending a public school has changed.  Previously there was a form that had to be completed and brought to the school, and then the school would send it to the Oklahoma Health Department for approval.  

 

Now, there is an online portal for obtaining an exemption through the Oklahoma Department of Health.  Even if you have obtained an exemption in previous years, you will have to go through the portal to apply for the current school year.  From our understanding, once you have applied, the approval process will take up to 3 days.  Once approved, you will receive notification and you will be able to download a certificate to bring to the school.

There will also be a link to the "Online Portal: Request an Immunization Exemption" that will take you to the online portal where you can create an account and submit your exemption request. There are instructions below the portal link that will explain the process.
